Carpe Diem Naha Membership Agreement
Article 1 Name and Location
The name of the gym is Carpe Diem Naha (hereinafter referred to as "this gym"), and it is located on the 1st floor, 3-61-9 Shuriyamagawacho, Naha City, Okinawa Prefecture.
Article 2 Purpose
The purpose of this gym is for members to maintain and improve their mental and physical health, and to promote mutual friendship among members through the use of facilities and instruction in jiu-jitsu and exercise.
Article 3 Scope of the Terms
Various regulations that this gym announces to members from time to time shall constitute a part of this agreement, and members will accept this.
Article 4 Changes to these Terms
This gym may change this agreement without obtaining the consent of the member, and the member consents to this.
Article 5 Notifications from the Gym
In addition to the preceding article, if the Gym deems it necessary, the Gym will notify the members of necessary matters at any time. In addition, the notice from this gym shall be the address and LINE group registered by the member with this gym and shall be sufficient.
Article 6 Notification of Changes
Members shall promptly notify the Gym of any changes to their address, contact information, or any other information entered in the membership application form.
Article 7 Membership
Those who wish to join the Gym shall acquire qualification as a member by accepting the terms and conditions, completing the prescribed application procedures, and paying the prescribed admission fee after obtaining the approval of the Gym. increase.
Article 8 Membership Requirements
This gym member shall fall under each of the following items.
1. Those who have been certified by this gym.
2.Those who have no health problems such as head injuries or infectious diseases and are suitable for exercising at this gym.
*In addition, members of organized crime groups, anti-social organizations, and other people who are not suitable as gym members cannot join. If any of these reasons are found to apply after enrollment, the enrollment will be cancelled. If you have a tattoo, please contact us in advance.
Article 9 Scope of use of this facility by members
Members shall be able to use the Facility on the days and times set by the Gym in accordance with the rules set by the Gym.
Article 10 Admission Fee
The admission fee shall be the amount determined by this gym and shall not be refunded under any circumstances.
Article 11 Monthly membership fee
The monthly membership fee shall be the amount determined by this gym and shall be paid by the prescribed method. No, if the monthly membership fee has not been paid for more than 3 months, we may request legal delay damages and request collection to an agent.
Article 12 Annual Payment of Membership Fees
The monthly membership fee shall be the amount determined by this gym and shall be paid by the prescribed method. No refunds will be given under any circumstances (advance payment, annual payment).
Article 13 Adjournment
Members may take a leave of absence with the approval of this gym by completing the procedure for a leave of absence due to the following public and legitimate reasons.
The deadline is the 10th of each month, and the application will be applied from the following month after completing the prescribed procedures. We cannot process adjournments by phone, LINE, or email. (Except for special cases, the maximum suspension period is 6 months, and 2,000 yen per month will be charged as a suspension fee.)
1. When it is determined that the member cannot enjoy the benefits of using this gym for a certain period of time due to overseas migration, transfer, etc.
2. When it is impossible for the time being to participate in general activities of this gym due to pregnancy, illness, injury, disability, etc., and the member has no intention to withdraw.
3. When the gym deems it a legitimate reason.
Article 14 Withdrawal
When members withdraw from this gym, they shall submit a withdrawal notice in writing.
The deadline is the 10th of each month, and the prescribed procedure is followed to withdraw from the following month. The membership fee for the month of withdrawal shall be collected in full even if it is in the middle.
In addition, in the case of notification after the 10th, it will be applied from the month after next and the membership fee will be paid as the membership fee before the procedure.
Cancellation procedures cannot be performed by phone, LINE, or email.
Article 15 Suspension of Membership and Expulsion
The gym may suspend membership or expel a member if any of the following items apply to the member.
1. When it turns out that the Membership Applicant does not exist.
2. When membership is being suspended due to violation of the terms and conditions at the time of application for membership.
3. When you have been expelled from this gym for violation of the rules in the past.
4. When there is a false statement in the items to be declared at the time of application for membership.
5. When you have neglected to pay the usage fee of this gym or have neglected to pay in the past.
6. When you slander this gym or seriously damage the honor of this gym.
7. When you can not contact for more than 2 months even if you notify the member registration address.
8. When you violate any of these Terms.
9. When you participate in a practice or match without notifying the gym.
10. When it is recognized that you have caused physical or psychological nuisance to other members, and there is no improvement even after warning.
Article 16 Usage Calculation Commencement Date
The usage calculation start date of this gym will be the contract start date from the date on which the prescribed procedure is performed.
Article 17 Usage Precautions
Members shall not engage in any of the following acts when using the Gym.
1. Acts contrary to public order and morals.
2. Acts that lead to criminal acts.
3. Acts that infringe on the copyrights of other members or third parties.
4. Acts that infringe on the property or privacy of other members or third parties.
5. Acts that slander other members or third parties.
6. Acts that interfere with the operation of this gym or damage the trust of this gym.
7. Violence that does not hijack sportsmanship.
8. Other acts against the law.
Article 18 Responsibilities
1. The gym does not take any responsibility for theft, troubles between members, injuries, etc. that occur in the gym.
2. If a member causes damage to this gym in violation of this agreement or other regulations stipulated by this gym, he/she shall compensate for all losses, costs, expenses, etc. caused by it.
Article 19 Self-responsibility principle
1. If a member causes damage to another member or a third party when using the facilities and services of this gym, the member shall resolve the matter at his/her own responsibility and shall not cause any inconvenience or damage to this gym.
2. This gym does not assume any responsibility for any damages incurred by members when using this gym's facilities and services, and is not obligated to compensate for any damages.
Article 20 Temporary closure and suspension of facilities
This gym may suspend or close all or part of the facility for the following reasons.
1. When it is determined that the opening is not appropriate due to unforeseen disasters such as natural disasters.
2. When inspecting, repairing or renovating facilities.
3. When enactment, revision or abolition of laws and regulations, administrative guidance, significant changes in social conditions, or other unavoidable circumstances occur.
4. When a player affiliated with this gym participates in a competition, when an event of this gym is held, or when this gym deems it necessary to take a leave of absence.
Article 21 Personal Information
We will strictly manage personal information collected from members and take appropriate preventive and corrective measures against unauthorized access, loss, destruction, falsification and leakage of personal information. In addition, we will properly educate and supervise this gym, and thoroughly manage and properly handle your personal information.
Please note that the member's personal information may be used for notification of sudden schedule changes, events of this gym, martial arts matches, and other solicitations.
